Please print and fill out the following forms. They should be turned in to your team mom prior to the first day of practice.
2025 Absentee Form2025 Player Contract & Parental Consent Form2025 Medical History FormYou will also need to provide (in Fall):
1. Birth Certificate - A copy will be fine, no original required.
2. Report card - End of last school year with all 4 quarters (copy).
Physical - There is NO NEED for a physical and physician sign-off this year. All you need to do is make sure the 2024 Medical History Form (link included above) is filled out completely and includes your insurance details.
If you are an adult and volunteering in any capacity (Coach, Team Mom, Team Doctor etc.) for the 2025 season you will be required to fill out a volunteers form which will be emailed to you prior to the season.
